Usage Note

Alienado can describe social estrangement ('feeling alienated from society') or, in older psychiatric usage, someone who has lost their sanity — hence the archaic term manicomio was once called casa de alienados. The reflexive verb alienarse means 'to become alienated'. The noun form is alienación.
